The Keweenaw Health Foundation proudly invests in local programs that promote health, wellness, and access to care.

Eligibility

Organizations eligible to apply include nonprofit entities, schools, community groups, and health-focused organizations serving residents of the Keweenaw region. Funding priority is given to programs that:

• Improve access to care

• Address health disparities

• Promote prevention and wellness

• Support innovative health solutions

Application & Instructions

Applicants must submit a completed application along with supporting documentation by the stated deadline. Instructions outline required materials, funding criteria, reporting expectations, and submission guidelines.

Review Process

• All applications are reviewed by the Foundation’s Grant Committee. Proposals are evaluated based on community need, measurable impact, financial sustainability, and alignment with the Foundation’s mission.

• Final funding decisions are approved by the Board of Directors.

Requirements if Awarded a Grant

Organizations receiving funding must:

• Sign a grant agreement

• Submit a final impact report

• Provide updates or photos when applicable

• Acknowledge Keweenaw Health Foundation support in communications
